Origin
Flexor retinaculum and tubercle of the trapezium (superficial head); trapezoid and capitate (deep head)
Insertion
Lateral side of the base of the proximal phalanx of the thumb
Nerve
Superficial head: recurrent branch of the median nerve; deep head: deep branch of the ulnar nerve (C8-T1)
